The incumbent is assigned to the Nursing and Patient Care Service. The Admissions/Discharge Coordinator will be responsible for the oversight and implementation of all processes related to admissions and discharges within the Community Living Center. The Admissions/Discharge Coordinator will also serve as back-up for the Resident Assessment Instrument (RAI), Minimum Data Set (MDS) and Restorative Care Nurse (RCN) Coordinator. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
