The incumbent serves as the Program Manager and Coordinator for the Transition Care Management Office, and assumes all of the technical and administrative responsibility for management of this program. This includes the OKC VAHCS and all CBOCs affiliated with the OKC VA HCS. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: Senior Social Worker, GS-12 Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. AND Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. AND Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:1. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.2. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.3. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.4. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.5. However, as this position requires a licensed professional, the physical demands remain equivalent to the clinical requirements for the position. The clinical component of this position provides direct services to patients and their families through casework. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing, clear distinctive speech, and manual dexterity. Manages the daily operation of the TCM program and provides accountability for program effectiveness and modification of service patterns to enhance customer services satisfaction. Provides administrative oversight of the TCM Program to include planning, establishing, and implementing policies and procedures, developing program goals and objectives, and monitoring, operating, and evaluating the functioning of the program. 2. Supervises all TCM Lead Coordinators and the Transition Patient Advocate embedded in the TCM Program. 3. Follows Veteran Health Administration (VHA) Policy requirements and develops local systems to identify and track TCM Veterans serviced by the facility and community. This may involve use of DOD Registry, Joint Patient Tracking Application (JPTA) - Veteran Tracking Application (VTA), Patient Treatment file searches, VHA Information System and Technology Architecture (VISTA), Care Management Tracking and Reporting Application (CMTA), FCMT, ICP, CPRS, NPEC, and any other data reports, classification reports, patient data exchange, and contacts with the community. 4. Promotes and coordinates evaluations for the TCM Program anywhere within the facility and outlying CBOCS in the OKC VA HCS catchment area to include, but not limited to Lawton, Ardmore, Altus, Ada, Stillwater, Enid, Blackwell, and Wichita Falls, TX. Establishes collaborative relationships with physicians, psychologists, social workers, nurses, and any other disciplines to foster their involvement in the care of Veterans in the TCM program. 5. Serves as a point of contact for all inquiries regarding transitioning Service Members to the VHA system. These duties include a wide variety of stakeholders (Veterans, families, Federal Agencies, State Agencies, community agencies, Veteran Service Organizations, Military Support Programs, etc.). Incumbent ensures that all Congressional Inquiries, PATS, and any other forms of customer service needs are addressed. 6. Responsible for management of Military Treatment Facility referrals for all Veterans located in the OKC VA HCS catchment area. This includes entering, editing, compiling, analyzing, and updating pertinent data. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Develops procedures for referrals between OKC VA HCS and MTF, as well as other community based hospitals that promote the appropriate and timely inter-facility transfers. 7. Prepares program reports for Social Work Service, OKC VA HCS Pentad and EL T (Executive Leadership Team), as well as VISN POC, and VACO Program Representatives. Ensures Performance Measures are met by the facility and all CBOCs on all measures pertaining to TCM Veterans. 8. Develops policy and procedure to ensure compliance with Joint Commission on Accreditation of Healthcare Organization (JCAHO), Commission on Accreditation of Rehabilitation Facilities (CARF), and· any other accrediting bodies and regulatory standards as needed and as the pertain to the TCM Office. 9. Establishes performance standards of the TCM Program that promote quality and efficiency of service to the Veterans in the TCM Program with overall goals of the OKC VA HCS. Responsible for all funds allocated to the TCM program, to included but not limited to FCP for salaries, and special programming needs or initiatives. 10. Manages the daily operation of TCM Program, identifies any gaps in service and develops a plan in consultation with appropriate VA Staff and . Community Stakeholders to address identified needs. Utilization of the resources available and maximizing of those resources to ensure management of TCM Program. The incumbent is responsible for the overall coordination of the Post Deployment Clinic, and has IDT components, poly trauma, mental health, and other VA IDT stakeholders. Provision of Clinical Services and Oversight of care management services 1. TCM Program Manager also functions as social work member of the IDT TCM Treatment Team, and will have caseload requirements. Ensures that all TCM Veterans are screened for case management and that those in need of services are assigned accordingly to appreciate clinics, particularly those that are severely ill or injured. 2. TCM Program Manager will complete psychosocial evaluations, care needs assessments, re-assessments, suicide risk assessments, treatment plans with Veteran centric goals of care, identification of psychological needs, and provide recovery focused care planning.
