The Pulmonary Nurse Case Manager is primarily responsible for caring for people who are sick, injured, convalescent or disabled. Under general supervision, they deliver care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation, in accordance with established philosophy. RNs collaborate with other professional disciplines to ensure effective patient care delivery and achievement of desired patient outcomes. The Pulmonary Nurse Case Manager will provide direct care and support for patients suffering from acute and chronic obstructive pulmonary disease. They are responsible for facilitating pulmonary rehabilitation procedures, case management, moderate sedation administration, care coordination and pulmonary infusions as directed via physician's orders. The nurse at this level will perform as the charge nurse who will direct the RNs and other subordinates in the unit when designated.
