This Social Worker - Residential Rehabilitation Treatment Program position is located in the Mental Health Service at the Tomah, Wisconsin VA Medical Center and assesses and treats the complicated psychosocial problems of Veterans. The Mental Health Residential Rehabilitation Treatment Program Social Worker is assigned within the General Domiciliary. MH RRTP Social Worker will have an extensive knowledge of Veterans and community programs/ resources, which will benefit Veterans enrolled in the MH RRTP. Incumbent is required complete initial psychosocial assessments, clinical notes, as well as, discharge planning and aftercare case management, and interact not only with the Veteran but also family and significant others, when indicated, community agencies and other staff. The Social Worker at the full performance level of the GS-11. Duties include but are not limited to: -Provide a full range of services within the Social Work Scope of Practice based on Veteran needs, acuity level, and preferences for care. Services may include, but are not limited to, the following: case management; individual, family, and group therapy; discharge planning; resource linkage; and care coordination. -Provide mental health and psychosocial assessment and diagnostic services for Veterans with a broad range of mental health issues, including services for patients with a myriad of mental health and psychosocial needs requiring diagnostic and intervention skills. -Provides education to patients and family members regarding mental disorders and best practices approaches to treatment. -Provide consultation and psychoeducation to Veterans and Veteran families on topics including community resources, Advance Directives, VA benefits, budgeting, activities of daily living, and recovery goals. -Provides consultation to health care providers about assessment findings and treatment plan for patients to ensure that care is coordinated, collaborative, and integrated within the Medical Center. Take a lead clinical role in case consultation. -Develops, implements, and documents the mental health treatment plan for an assigned caseload of patients, as indicated and appropriate for the specific job setting and duties of the social worker. -Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs, and community agencies. -Possesses general knowledge of Veteran's benefits and services relating to special programs, service-connected compensation, and non-service-connected pension. -Fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with NASW Code of Ethics, VHA Code of Ethics, and local ethics policies. This includes but is not limited to: compliance with all laws and regulations related to confidentiality and release of information; protecting Veterans' personal identifying information. Work Schedule: Monday through Friday, 8:00AM to 4:30 PM, alternate tours or weekend hours may be required to meet patient care needs. Financial Discloser Report: Not required
