Rehab 3 has a RN Clinical Care Coordinator(CCC) position available who provides patient care. The CCC works with another CCC as charge nurse and coordinates care for 19 bed rehabilitation unit. The unit is Commission on Rehabilitation Facilities (CARF) Accredited for Comprehensive Integrated Inpatient Rehabilitation Program (CIIRP) The unit provides rehabilitation services for stroke, debility, and amputee patients for active duty military and eligible veterans throughout the US. Clinical Care Coordination for the CIIRP. This program needs an RN who will work in collaboration with the Medical Director and members of the rehabilitation interdisciplinary team (IDT) in a leadership role. The IDT is working on new CARF accredited program for stroke. In addition to patient care requirements, the RN would assist in leading team meetings, documentation, and making patient care rounds daily. Duties include, but are not limited to: The Clinical Care Coordinator alternates with other Clinical Care Coordinator in charge nurse role for the unit. Familiarity with CARF standards is desirable and active involvement in the further development of the CIIRP and Stroke programs. The position reports to the Nurse Manager of the Inpatient Rehabilitation Unit. Work Schedule: Working hours 10 hour shifts Monday through Friday 0600 - 1630 with one day off a week in conjunction with the patient work load with some weekend/evenings to meet staffing needs.
