The position of HCHV Case Manager is accountable for fulfilling the responsibilities of two separate yet interrelated roles, namely Case Manager and Outreach Clinician. The program's scope of responsibility may range throughout the West Texas VA Health Care System (WTVAHCS) catchment area. Performs specialized treatment of complex physical or mental illness. Incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Provides individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with poly-trauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, addictions, and other serious disorders. Offers consultation to colleagues and students on the psychosocial treatment of Veterans treated in specialty areas, rendering professional opinions based on experience and expertise. Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications as assigned. Provides HCHV services as outlined in VHA Handbook 1162.09 and appropriate law and regulations, ensuring coordination of HCHV services with other homeless programs at the West Texas VA HCS such as HUD-VASH, VJO, CEC and SSVF. Collects and submits HCHV program participant data, as outlined by NEPEC, HOMES, and VSSC evaluation procedures. Works with HCHV clinical staff to encourage timely reporting of HCHV program activity as needed. Provides full range of case management services , outreach (to encampments, shelters, drop-in centers and other community locations) and clinical services (psychosocial evaluations, diagnosis, psychotherapy, supportive counseling, recovery/treatment planning, discharge planning, Suicide Risk Assessments and Safety Planning in accordance with policy, etc.) to the identified population. Utilizes established process for referrals, screenings, entries, and exits to all CERS for which HCHV provides oversight. Ensures the HCHV program obtains and maintains a third party accreditation with Commission on Accreditation of Rehabilitation Facilities (CARF) and, where indicated, The Joint Commission in accordance with VHA Handbook 1170.01, Accreditation of Veterans Health Administration Rehabilitation Programs. Independently conducts psychosocial assessments; develops treatment plans in collaboration with the Veteran/family and the interdisciplinary treatment team. Represents the HCHV Program both within the VA health care system and the community at large. Collaborates with community agencies and other VA programs to develop and implement services for homeless veterans. Completes all reconciliation necessary to verify bed days used at CERS Contract Sites. Provides Case Management services, groups, and referrals to Veterans enrolled in CERS Contract Sites. Manages and takes responsibility for a full caseload of Homeless veterans in the HCHV program. As an Outreach Clinician with the Homeless Veterans Program, the incumbent will manage a full clinical workload and further engages in psychosocial/diagnostic assessment with patient/family on each active case. The Case Manager demonstrates ability to establish therapeutic rapport, demonstrates effective interviewing skills, and elicits relevant information by case presentation and observation. The HCHV Case Manager initiates outreach to the community to identify HCHV veterans. The target population consists of veterans who are homeless, and who are chronically mentally ill, including substance abuse. Provides therapeutic interventions that are consistent with the psychosocial assessment. Incumbent demonstrates the ability to conceptualize, communicate, prescribe and provide intervention to patient and/or family. Interventions must be consistent with clinical privileges. Ensures continuity of patient care in the development and implementation of the plan of care. Facilitates evaluation and treatment of veterans in HCHV contract facilities and participates in case conferences with those veterans; plans will reflect continuity of care including expected short and long-range outcomes. Provides all necessary direct and indirect patient care services consistent with area of assignment. The incumbent is authorized to obligate funds for payment of contract services, and provision of emergency funds to veterans in treatment programs. The incumbent demonstrates the ability to balance quantity and quality requirements. In most instances, agreed upon priorities are maintained, and caseload is effectively managed while keeping the HCHV Program Manager informed of problem areas.
