Recruitments/Relocation Incentive may be offered to highly-qualified applicants. Major Duties/Responsibilities Primary care management in the Outpatient Geriatric Primary Care Clinic settings. Conduction and documentation of Inpatient Geriatric Consultations. Completion and documentation of follow-up evaluations in the Community Living Center. Assessment, development of treatment plan, and documentation associated with Community Living Center (CLC) admissions. Assessment of needs and coordination of services associated with Community Living Center (CLC) Discharge Planning. Placement of orders, including interpretation of diagnostic testing and therapeutic modalities, including but not limited to laboratory tests, EKGS, radiological studies, hemodynamic monitoring, treatments, IV fluids and electrolytes, oxygen. Placement of pertinent consults, devices, supplies, and durable medical equipment. Patient care as outlined in clinical privileges via means of telehealth. Provision of counseling and education to patients, families, and caregivers related to various clinical issues. Work collaboratively with other health care disciplines as part of the team-based model of care, including nursing, rehabilitation, social work, mental health, pharmacy, and dieticians. Evaluation of clinical changes. Prescription of medications in accordance with VA regulations. Completion of appropriate forms for various services, as indicated (e.g. Aide and Attendance). Obtain Informed Consent and document Informed Consent for any procedure for which the APRN is approved to perform. Active participation in the Community Living Center Interdisciplinary Team meetings. Active participation in the Community Living Center morning screening and admissions meetings, including review of new consults. Assessment and documentation of the Veteran's or appropriate surrogate's directions for life-sustaining treatment. Develop and promote educational initiatives for members of the SLVHCS GEC interdisciplinary teams and other healthcare employees. Oversee medical students, residents and fellows in different healthcare settings. Other duties as assigned. Work Schedule: Flexible Work Schedule, 8:00 a.m. - 4:30 p.m. On-Call Responsibility - Participate in Geriatrics and Extended Care (GEC) Service call - after-hours and weekends, including, but not limited to Community Living Center (CLC) call, Home-Based Primary Care call, and Hospital In Home call...five to seven on-call per year.
