DUTY STATION: The duty station is at the Jefferson Barracks Division however, the duty station may be changed to one or multiple associated VAMC locations within the metropolitan St. Louis, MO area based on the needs of the medical center. If you are not a permanent VA-Wide Agency Employee, then you must apply under CBSS-11092542-21-EG The incumbent serves as Social Worker for the Substance Abuse Residential Rehabilitation Program (SARRTP) and Aftercare Program in the Mental Health Service (MHS) at S. Louis VA. The work usually takes place at the SARRTP Jefferson Barracks location but may also take place in the community, CBOCs, acute inpatient wards and clinics where veterans are referred for aftercare service. THE FUNCTIONS INCLUDE, BUT NOT LIMITED TO: Coordinates treatment services and discharge planning for the SARRTP and Aftercare Programs. Plans, schedules, and coordinates work to solve problems related to the provision of high-quality substance abuse treat. Responsible for providing an appropriate treatment environment for veterans treated or co-managed by SARRTP and/or aftercare who suffer from substance abuse addiction, chronic mental illness, homelessness and unemployment. Coordinates referrals to community service providers. Participates in treatment team planning and provides social work viewpoint to SARRTP team and/or agencies in the community. Responsible for services in compliance of all; performance measures, Advance Directives, MCM's, SOP's, VA and VACO Directives, CARF and The Joint Commission. Responsible for workload documentation & reports. Provides accountability to ensure maximum benefit and utilization of the 25-bed residential unit. Clinically, the incumbent will be directly responsible for facilitating groups within the program. Assist in screening, assessing and evaluating veterans for admission into all levels of substance abuse care/counseling in the program. Work closely with referring programs and provide education/resources when referrals are delayed or not accepted.
