The North Florida/South Georgia Veterans Health System is recruiting for one (1) Registered Nurse (PCMHI) Care Manager position in Gainesville, FL for the Mental Health Service Line. The NF/SG VHS is affiliated with health care programs at local colleges/universities. Florida has a low cost of living and no state income tax. Enjoy excellent recreational activities year-round at nearby beautiful beaches. Incumbent must be able to make sound clinical decisions, practicing at the high end of their license. Incumbent must thoroughly understand Mental Health Nursing. Incumbent must possess excellent interpersonal skills. Incumbent must possess fluency in written and spoken English. Incumbent must possess computer literacy. Incumbent must possess excellent customer service skills. Observes, recognizes, and reports safety hazards. Must maintain competency in BLS. Identifies and refers patients in imminent danger of hurting self or others as per established PCMHI memorandum and hospital policy. Incumbent must possess high degree of self-direction and be able to work independently. Maintains an updated care management panel of patients. Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendation to the patient's medical provider. Track patient follow up and clinical outcomes using a registry. Documents in-person, telephone and virtual encounters in the registry and use the system to identify and re-engage patients. Documents patient progress and treatment recommendations in HER and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers. Facilitate treatment plan changes for patients who are not improving as expected whom might need more intensive or more specialized mental health care, in consultation with the medical provider and the psychiatric consultant. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Develop and complete relapse prevention self-management plan with patients who have achieved treatment goals are soon to be discharged from care management. Maintains patient confidentiality and privacy in accordance with HIPAA and PCMHI policy. Serves as a patient advocate and educate patients and families as clinically indicated. Possesses knowledge of hospital and PCMHI policies. Demonstrates ability to collaborate and communicate effectively in a team setting. Ability to maintain effective and professional relationships with patient and other members of the care team. Assists in gathering data for QI/PI processes. Promotes PCMHI care management to all sites in NF/SG. Occasional travel, you may be expected to drive to all clinic sites within NF/SG. Performs other tasks as assigned. Employment contingent on completion of pre-work for training (Phase 1) and VHA PCMHI COMPETENCY TRAINING (Phase 2) within first year of employment and passing Phase 3 Testing (Three months post training) and (Six Months post training) with no more than 3 attempts each. Successfully managing a panel of 90-120 Veterans per month within 6 months of start date to PCMHI Care Manager position.
