Work Hours/Shift:

Full Time, Days, fixed hours/points per day. 

GENERAL SUMMARY:

Functions as part of the interdisciplinary team providing rehabilitative services for qualified homecare patients.  The therapist assists the physician in evaluating level of function, helps develop the plan of care (revisiting it as necessary), implements the plan of care, prepares clinical and progress notes; advises and consults with the family and other agency personnel and participates in inservice programs

You will be responsible for:

• Functions as part of the interdisciplinary team providing rehabilitative services for qualified homecare patients. 
• The therapist assists the physician in evaluating level of function.
• Helps develop the plan of care (revisiting it as necessary).
• Implements the plan of care.
• Prepares clinical and progress notes.
• Advises and consults with the family and other agency personnel.
• Participates in inservice programs.

What will you need?

• Bachelorâ™s degree in Physical Therapy
• Current state of Florida Physical Therapy license
• Current Florida driverâ™s license
• Basic Life Support certification
• Demonstrates moderate computer skills
• Previous home health experience Preferred
• Knowledge of regulatory requirements Preferred
• At least 2 years of clinical experience as a physical therapist Preferred
• Experience working with adult and geriatric rehabilitation cases Preferred
• Home health experience within the past 3 years Preferred
• Current knowledge of homecare requirements and OASIS assessments Preferred
