We are currently looking for a Social Worker MSW, Case Management to provide direct clinical social work services to patients and their families in a multidisciplinary team approach. The incumbent will also be accountable for the following duties:
Complete an initial psycho-social assessment, including social work treatment plan.
Provide individual, family or group counseling, complex discharge planning, and bio-ethics case consultation.
Make referrals to community agencies and entitlement programs.
Ensure continuity of care for those patients with complex post-acute care needs.
Maximize patients and their family abilities to meet the continuing care needs within their social and financial framework.
Understand stages of growth and development to assess each patient s age specific needs to provide age specific treatment and care.
Document interventions in patient s chart and submits monthly statistical information to the department.
Assist with hospital-sponsored support groups.
Function in a liaison role between the hospital and community in making community resources available to the patient and family.
Collaborate with all members of the healthcare team to develop, manage and communicate discharge plans.
Maintain current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term care and low-income patients
Able to facilitate the process for third party payers and/or managed care principles.
Knowledgeable of guidelines for Medicaid/Medicare and related state programs.
Aware of InterQual/MCG criteria and other guidelines for medical necessity, setting and level of care, and concurrent patient management.
Familiar with healthcare delivery system, utilization and review and case review procedures.
In possession of working knowledge of benefit plans; HMO, Medicare, Medicaid, Employee, Commercial, Medicare Advantage, etc.
Capable of working in a fast-paced team environment.
Strong in prioritizing and multi-tasking.
Great at making sound, independent clinical judgements and can remain professional under pressure.
A MSW (Master s in Social Work) degree.
Graduated from an accredited School of Social Work
State licensure required by the NJ State board of social work examiners.
Licensed social worker (LSW) or licensed clinical social worker (LCSW), depending upon years of social work experience and clinical course work.
Minimum two years' experience in an inpatient psychiatric program preferred but not required.
Excellent communication, interpersonal and time management skills required.
General knowledge of community organizations of modern principles of training and education; thorough knowledge of the causation, symptoms and effects of mental illnesses, proper patient evaluation and treatment needs and assessments of report writing.
Ability to establish and maintain effective working relationship with others; express ideas clearly and concisely; analyze problems and situations and recommend appropriate action; coordinate several projects concurrently.
