Providence is calling a Physical Therapist (PT) (Physical Medicine Dept, Day Shift, 1.0 FTE) to Providence ElderPlace in Seattle, Washington. This position will serve our clinics in the Seattle and Greater Seattle areas. We are seeking an experienced and independent PT to join our team of caregivers who provide assessment and treatment in both clinic and home based settings. PTs are an integral part of the Elderplace program, working to maintain participants with the highest level of independent mobility and ADL's in community settings.

In this position you will:
Provide initial and annual participant evaluations in accordance with PACE regulations.
Consult with the physician and an interdisciplinary team to perform PT evaluations, establish treatment plans with goals, provide treatment, delegate specific treatments and perform discharge activities.
Provide education to patients and their families.
Utilize software systems for the maintenance of patient records and communication with team members.
Use critical thinking skills, decisive judgment and your ability to make independent decisions.
Required qualifications for this position include:
Bachelor's Degree and a Graduate of an accredited school of Physical Therapy
Current Physical Therapy (PT) licensure in Washington State
Current Cardiopulmonary Resuscitation (CPR) certification
One year experience in atherapist capacity in a hospital, skilled nursing facility or otherrelated medical field
Preferred qualifications for this position include:
Post graduate experience
