Full time VA Nurses earn 8 hours of annual leave and 4 hours of sick leave every pay period. There are 10 paid Federal holidays. VA Puget Sound offers free onsite parking for employees or take advantage of generous transit benefits.
The Certified Wound, Wound/Ostomy, or Wound/Ostomy/Continence Nurse is an Advanced Practice Registered Nurse holding the certifications listed above and is responsible for program planning and management, delivering patient care utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation, and effectively interacting with patients and other health team members while maintaining standards of professional nursing. The manager's practice is characterized by leadership and expertise in managing various issues. Additionally, the incumbent acts as a consultant to outpatient, inpatient, resident, primary care, home-based primary care, medical and nursing staff in assessing, planning, implementing, evaluating and reassessing the care of patients with a variety of wounds and stomas, to include but not limited to lower extremity wounds, abdominal stomas, draining wounds, fistula, pressure ulcers, dermal wounds, acute surgical wounds, and incontinence issues, and is involved in the physical, emotional and social rehabilitation of patients. The incumbent is accountable for the 24 hour management of a nursing clinical area. The Nurse Manager's practice is characterized by leadership and expertise in managing various issues. When providing direct services to patients the manager acts as a role model of expertise in ethical and clinical wound care nursing. Through consultation, continuing education, and training both employees and patients in wound care strategies, the incumbent contributes toward a more knowledgeable workforce and effective plan of care. The manager demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
Duties include but are not limited to:
Responsible for care delivered to each patient demonstrating clinical nursing skills commensurate with the level of knowledge and education of a certified practitioner, ensuring it is in accordance with nursing practice standards and the mission, vision, and goals of the VA Puget Sound Health Care System.
Independently manages complex patients with complex wounds using evidence-based standards of care and national wound care guidelines.
Assesses and reassesses patient's condition and identifies patient needs utilizing the following: pertinent patient history obtained via interview techniques; direct observation and physical assessment/examination to include diagnostic screening techniques; and a review of the medical record.
Responsible for timely documentation in the patient records, maintenance of records, logs and preparation of reports, documents, statistical surveys and other data as required in various patient settings throughout the VA Puget Sound Health Care System.
Uses a variety of cues to obtain data on challenging patients, clinic, and program operations. Assists staff in accurately discriminating among multiple patient problems and/or systems issues and assists or resolves these.
Systematically evaluates current practices. Formulates outcomes for groups of patients, clinics, or the wound care program. Proposes revisions based on evaluation of data and outcomes.
Guides staff in the application of the critical thinking process. Analyzes documentation and reporting systems and plans strategies to initiate needed change.
Accepts responsibility and accountability for decisions related to the Wound and Skin Care Program.
Initiates strategies for policy development including writing and updating facility and program policies on wound care, pressure ulcer prevention, ostomies, and related issues.
Develops and maintains all practice-specific, specialized competencies and mandatory reviews for self and Wound and Skin Care Program staff
